You need the following information to register a death:

  • The full name of your loved one (and any previous names)

  • Their date and place of birth

  • Their last address

  • Their occupation (or last occupation if retired)

  • The full name, date of birth and occupation of their spouse or civil partner (if applicable)

  • The date and place of death

It is helpful to bring documents such as passport, driving licence, birth certificate, marriage certificate or NHS number if available, but these are not always essential
